Create a quiz in DocSurvey

This guide shows how to create and publish a quiz from the DocRead Cockpit application.


1. Access the quiz section

  • Open the DocRead Cockpit application.
  • Select Quizzes from the main menu.
  • Select Create Quiz.

2. Choose a quiz design

  • In Create Quizzes, select a design/template.
  • Click or drag and drop the selected design into the empty form area.

3. Add questions

Add one or more supported question types:

  • Radio Button Group
  • Checkboxes
  • Dropdown
  • Rating Scale
  • Yes/No

4. Configure each question

  • Enter the question name.
  • Add answer choices where applicable.
  • Set a correct answer for scored questions.
  • Mark at least one question as required.
  • Optionally set read-only and title/description visibility.

5. Set quiz details

  • Provide a quiz title.
  • Add a description.
  • Set a passing rate (for example, 80%).

6. Save and finalize

  • Select Save & Exit after configuration is complete.
  • Select Confirm to finalize.

7. Verify the quiz

Return to Quizzes and confirm the newly created quiz appears in the list.
